ASSUMPTION OF RISK, WAIVER OF LIABILITY, AND INDEMNIFICATION AGREEMENT

I acknowledge that participating in dance classes involves inherent risks including but not limited to: physical injury, muscle strains, sprains, broken bones, and other injuries that may result from physical activity and interaction with other participants.

I voluntarily assume all risks associated with participation and waive any claims against the dance studio, its instructors, employees, and venue owners for any injuries or damages that may occur.

I certify that I am in good physical condition and have no medical conditions that would prevent safe participation in dance activities. I agree to immediately inform the instructor if I experience any discomfort or pain during class.

I release and hold harmless the dance studio and its representatives from any and all liability for injuries or damages arising from my participation in dance classes.
